Happy New Years Eve

Our office will be closing at 2:00 pm today and reopening as usual on Monday at 9:00 am.
I wish you all a happy new year. If you need care during this long weekend, The after hours PERG starts to see patients at 2:00 pm this afternoon.
We have been quite busy today. There is a nasty cold and cough going around. Our nebulizers have been buzzing with all of the wheezy children in here.
If you have one of our loaner nebulizers please make sure to bring it back to the office next week.
We have some H1N1 in stock and will be giving a limited amount Tuesday, Wednesday and Thursday. Call those mornings to get on the "same day" vaccination list.
Shots are given every day to patients who are here on the main schedule seeing the doctors.
I will check back in on Monday to let you know what's been going around all weekend.

New Year, New shipment

Ok folks we got some vaccine in today! Multi dose vials (with a preservative, so a small amount of thermerisol) we can do shots for 6 months and up.

Please call for a shot appointment. We have _limited_ spots, 10 this morning, 10 in the afternoon. If you cannot get in today, don't fret. We will have some shot appointments for Wednesday Dec. 30th as well.

Recall did not affect us

As you may have read there was yet another recall of some batches of the H1N1 mist.
The doses we gave at this office were NOT among the batches that were affected.
This was not a safety issue, but rather that the units were possibly less than full doses. Patients who received the recalled doses do NOT need to repeat.

We are Not in the office this afternoon or tomorrow. As always, If you have a concern when our office is closed, please contact the after hours clinic at 387-9293

Bill Graham report

We just got a call from a parent who is working at the Bill Graham event.
As of 3pm there are reportedly NO lines!!!

traveling tips and stuff...

I hope everyone is getting their H1N1 needs taken care of from the opportunities out there in town.
Today is the big clinic at the Bill Graham and Care Practice still has vaccine as well.
Care Practice is doing drop in visits today from 4-5 and tomorrow from 3:30 -5.

I don't expect any vaccine here until January.

This week we are still seeing a tummy bug and lots of strep throat. We also have our share of typical winter colds and coughs.
Lots of our families are planning flight travel for the holidays. Here are a few tips.

* have tylenol and or Motrin/advil with you on the plane should you need it. It won't do you much good if it is packed away in checked luggage.
you don't need to give it unless you have a fussy child on your hands. Know your doses!

*have your infant sucking during take off and landing to help with ear pressure.
Older kids can suck on a lollipop or chew gum. There are little ear plugs available at drug stores called "earplanes" that are very helpful for problem ears

AYR saline gel is a nice thing to have on hand to help noses deal with the dry airplane air. This is great for all ages and is available over the counter right by the saline drops and sprays

* when you are calculating your need for diapers and snacks, be a pessimist and count on significant delays...there is no down side to getting to your destination with extra diapers on hand. the alternative is NOT pretty.

For long flights with a lap baby, if you have room in your diaper bag, pack an easy change of clothing for you as well as the baby ( this is personal experience talking...6 hours covered with poop from a leaky diaper, 22 years later and I remember it well)

It is always a good idea to know what your care options might be if your child gets ill while you are away.
Check with your insurance company to see how they like to handle those situations.
Find out what urgent care options will be available to you near where you are staying

Monday morning

NO vaccine here today.
We did get a message from the health department suggesting that january promises PLENTY of vaccine for anyone who wants it. Perhaps even Target and Walgreens will get their supplies soon.

Remember that second doses need to be at least 21 days apart, but there is no maximum time as long as the second one is given this season.

Check the earlier entries for info on where you can get the vaccine this week.

We just got word of a new H1N1 opportunity!

Childrens Hospital of Oakland is offering a FREE H1N1 vaccine clinic for all children. They will also vaccinate any family members of high risk patients.

There seem to be more and more opportunities out there to get the vaccine, and who knows, maybe we will get another shipment here next week.....or in March when nobody wants it anymore!!

Just as an FYI, one of our moms went to Care Practice yesterday for pediatric vaccine. On the phone they were told that it was available, but when they got there the story changed and they ended up with a shot from the multi dose vials.

We are not stressing about the tiny amount of thimerisol. The Vaccine from the multi dose vial is a fine choice if that is what is available

Thursday update

We are not holding any more shot clinics for H1N1 until we get future shipments.
As of this morning, Care Practice says they still have some mist as well as some of the preservative free pediatric vaccine (their vaccine was not impacted by the recall either)
The hours for shots there are are on a drop in basis Monday through Thursday 4pm -5pm
The huge vaccine clinic at the Bill Graham auditorium on the 22nd is also an option for anybody from 6 months through 24 years.
CPMC clinic is an option for people 4 and over.

We will update the blog when we get our next shipment, but we will be strictly limiting how many people we can accommodate here in one session. We haven't figured out the magic formula for trying to be sensible and fair while still satisfying the needs of all or our worried parents.

In the meanwhile, we have NOT had much fever going on this week. The big deal going around has been a tummy bug. Lots of vomiting kids out there. Thankfully it hasn't been lasting for more than a few days.

Wednesday H1N1 Availability

Looks like we have some _Mist_ left for Wednesday. Please call for available times in the AM.

There have been some complaints about the waiting time for flu shots at the office. Just remember folks, we have a fully booked schedule for all of our staff members (doctors, nurses, MA's and front office staff) _before_ we add the extra shot appointments. Please come with happy, patient hearts. The alternative is to heavily limit the number of shots we give, meaning only a handful of people a day could come in for the H1N1 vaccination.

Special clinic at CPMC

CPMC just announced that it will be holding a special clinic to give the H1N1 vaccine to Pregnant women, anyone who gives care to a small child ( including expectant dads) and children over 4.
This clinic will be held on December 21st 10am-7 pm
3700 California Street lobby level conference room C

The vaccine they will be using is the Novartis brand, multiple dose vial (which means there is that pesky trace of thimerisol)
This brand is approved for children over 4 only.

Monday update

We still have plenty of mist for the kids over 2 and some of the shots available.
If you want one of our doses, Please call tomorrow to be put on the Tuesday shot list. Care Practice says they still have vaccine as well. Their number is 864-4444.

We did get an e-mail from the health department advising us that there is NOT a lot of the preservative free vaccine available. Most likely we will continue to get the multiple dose vials. Waiting for the preservative free may not be your best option.

They do report that there is still quite a bit of the H1N1 virus around (although 1 in 6 people have already had it!) For the most part it is fairly mild.
Any fever lasting more than 4 days, or any significant coughing should be evaluated.

Most of my Monday morning calls so far have been the general coughs and colds.

Ok the phones are ringing off the hook, everyone wants their shot TODAY!
Sorry folks just not possible. The staff has already over-extended our time and space to accommodate people today. There is H1N1 flu mist available for healthy kids over 2 at a practice called Care connection (found by one of our enterprising moms)

They are giving the mist on a walk in basis from 3:30-5:00 pm Monday through Thursday
(so no appointments there today either, but they are doing a clinic on Monday!)

I called there today to check out their stock. They have a lot of the mist and a little bit of pediatric shots.

Please keep in mind, we do need to vaccinate our high risk kids as well as our general population. If you are told, "no we cannot get you your shot this shipment," please do not take it personally. We love your child, just as much as our high risk clients. We are trying our very best to get everyone their shots in a timely manner.

Vaccine for NOE Valley Pediatrics patients only

My oh my but the internet is a powerful tool. It didn't even occur to me that folks other than our patients are checking out this blog, but it turns out they are.
At this point we are trying our best to get our existing patients vaccinated and we do NOT anticipate having any extra vaccine for non patients.

WE GOT SOME VACCINE

As soon as I posted the last post we received a shipment.
We just got 100 doses of the H1N1 mist.
this is for healthy folks aver the age of 2

We also got 100 doses of the multiple dose vials.
This is okay for anyone that wants it.
It does have a tiny bit of thimerisol.
Children under 3 get .25 cc
Folks over 3 get .5 cc

We do have more preservative free on order but trying to figure out when we will get it more feels akin to calculating the odds at the poker table I just left in vegas!

If your child is due for a first dose or booster and you want one of these doses, please call the office to schedule when you can come it!

This is Nurse Judy signing back in.
Thanks to Charity for keeping the blog going while I was gone.
While I was away we were notified that we would be getting TEN doses ( not boxes...DOSES!!) of the H1N1 this week.
Charity wasn't sure whether to laugh, cry or scream. Regardless of that e-mail alert, we have not received anything as of yet...not even the ten.
We have to date gotten only a tiny percent of the amount we ordered.
The order was placed as soon as we were allowed to place it, and was for as much as we were allowed to order. We were permitted to request the same amount as our existing seasonal flu order.
Why some practices have gotten what they asked for, while we have been frustrated by lack of vaccine is a question that I have not been able to get answered.
In the meantime...the opportunity to get vaccinated on the 22nd my be the best option.
I will update as soon as there is a change in our status

Monday Morning Blues

Hey folks. Well no sign of H1N1 this morning. So no news, is no news.

Last week was busy with a lot of calls about nausea and vomiting, there is a tummy bug going around. As always your best protection is careful frequent hand washing. And the biggest problem is dehydration. Here are some tried and true tips:

Keep your child hydrated with small frequent sips of a clear liquid.

Best liquids-
Breastmilk
an electrolyte replacement like pedialyte
juice diluted one to one with water
broth or stock (chicken, beef, vegetable- a good choice for kids who are refusing sweet things)
popsicles
jello (best to pick ones light in color, avoid red- it stains!)

If they hold that down for a while and seem hungry it is ok to move onto the BRAT diet.
Bananas
Rice (white rice or rice cereal)
Apples (sauce, not chunks with skin)
Toast (white bread toast, no butter)

avoid dairy except for yogurt. avoid the "p" fruits- peaches, pears, plums, and prunes. If the stools are very runny best to hold off on the cows milk. Try rice, almond or soy instead. Breastmilk is ALWAYS fine.

If they do well with those offerings plus the clear liquids they can move onto a bland diet. Nothing acidic or spicy (avoid tomatoes). Examples: Chicken noodle soup no vegetables, poached eggs on toast, broiled chicken, flavored rice, classic sick bed comfort foods.

Thursday

We just received 100 doses of SEASONAL flu mist for anyone interested.
We didn't think that any more would be available this season, so I was delighted that we were able to get this.
We still have no idea what the regular seasonal flu has in store for us later this winter.

I am still seeing some patients with moderate fevers and also seeing some folks with several days of diarrhea.

I have seen what I suspect are a couple of kids with "slap cheek" virus.
For the most part it has been relatively quiet for the first week of December.

STAY Healthy...I am going to pass the Blogging over to Charity until the end of next week ( Lucky Nurse Judy is going away for a few days....)

upcoming vaccine clinic

I just got word from the SF public health department that they will in fact be having another H1N1 vaccination clinic at the Bill Graham Auditorium on December 22 from 10am -7pm

This will be for anyone considered high risk. This includes all children from 6 months through 24 year of age.
They will give the second dose to anyone who has had the first one prior to December 1st.

Hopefully we will have our vaccine before then, but this is good news.

Wednesday update

The best info that I was able to wring out of the "powers that be" yesterday, is that they are expecting another shipment of the H1N1 vaccine the second week of December. Once they get it, they will attempt to allocate it fairly. ( Does that mean we get a reasonable amount this time? I HOPE so!!)
San Francisco health departments are NOT planning any more vaccine clinic days at this time.
I am getting many calls from parents about the second dose. For those of you lucky enough to have gotten that first dose, I understand that the minimum 3-4 weeks has passed and it is time for the booster.
I spoke to the manufactures as well as the health department to get as much understanding as I could about the timing.
Everyone I spoke to said that while there is indeed a minimum wait time in between the doses there is NO maximum. The first dose primes the body and in some cases gives adequate protection. The second dose boosts this protection and can be given any time within the flu season.
Ideally of course we would offer it in a more timely manner, but don't stress if it is delayed.
It is also fine to mix and match types of vaccine. If your child is over two they can boost with the mist even if the first dose was the injection.

December already?

Several things....
We still have not gotten notification of upcoming shipments this morning.They are supposed to send an email the day before to alert us that the vaccine is on the way

Of course, the last shipment took me by surprise and came with no warning, so our status could change quickly.

We have gotten several questions about reserving a vaccination and getting on a list.
We are not doing reservations. If we were, I am quite sure that all of our upcoming vaccine would already be spoken for by the lucky few that camped on the phone lines here. Until our supplies are truly adequate we want to put more thought into the process than a simple first come first served.
There are 2 possible "lists".
The first is Josie's list of high risk patients. She has kept this for years and uses it to alert patients every year when the flu vaccine becomes available. If you have a child that you believe is high risk for any reason, do talk to us about making sure you are on that list.
The second possible list would be the daily log of people scheduled to come that same day for available flu shots. Once we have the vaccine in stock we will be doing flu shot clinics Tuesday, Wednesday and Thursday 10-11:30 or 2-4.

What's going around?...Yesterday was the Monday after a 4 day weekend and I anticipated that it would be much busier than it actually was, which means things are fairly quiet out in the illness world.
We did have a handful of kids with several days of moderately high fever.
We are also seeing a bunch of patients with a mild tummy virus that comes and goes fairly quickly.
I will post again tomorrow unless we get a shipment today.

Happy Thanksgiving

Nothing new to report.
We are giving the baby H1N1 to our highest risk young patients.
On Monday we will surely not receive vaccine but will possibly get notification about our next shipment.
I will keep the blog updated as soon as I hear anything.

As far as the neighborhood update I am talking to quite of few parents frustrated by the never ending mild cough.
We are seeing a bit of strep throat. ( fevers and sore throats should be checked out, if there is congestion in the mix that makes it less likely that it is strep)
There are also quite a few wheeze rs out there.
If you think your child might be wheezing please do NOT wait until Monday to be checked out! Call PERC if needed 387-9293

Tuesday update

We just received a tiny amount of vaccine. We now have 60 doses of the pediatric preservative free.
Because it is such a limited amount we are going to try to give this to those who need it the most.
IF your child is under 3 and has a history of wheezing and nebulizer use, call the office as soon as possible to get on the list for this allotment.
If you have a child under 3 who has a sibling who is less than 3 months old we would also consider you.
I Don't want this to be a feeding frenzy and we do expect to get more shortly.
Call if you think your child qualifies for this batch.

Monday update

We currently have no H1N1 vaccine.
According to the folks in charge we are still on the list but will NOT get any this week
Once we do have the vaccine it IS worth getting the second dose for any patients under 9 who have been lucky enough to get the first dose.
We still have plenty of seasonal flu for the under 3 year olds and a bit left of the regular seasonal flu injection.


It does seem a bit quieter as far as the "flu patients" go. Our phones remain busy, but today I have been talking to folks that seem to have typical colds and coughs.
There also seems to be a mild tummy virus going around.
I am also seeing several cases of head lice...

I will update next on Wednesday unless we get surprised by a shipment ( I am not holding my breath!

thursday update

Unless you have spoken to us and put your name on the list, we are now OUT of the H1N1 mist.

I will update the status tomorrow.

Please remember, although we would love to vaccinate all of our patients and families, none of the actual cases of H1N1 that we have dealt with in this office have been anything more than an annoying several days of typical flu symptoms!!

Thursday's update

This week we received 150 doses of the H1N1 mist. This is indicated for healthy folks over the age of 2. Severe asthmatics and pregnant women should not get this.
Most of these doses will be gone by the end of this week.
Hopefully next week or the week after we will get another shipment. I will keep this blog updated.

in the meanwhile let me address some of the more frequently asked questions.
Q. What's with the thimerisol?
A. years ago, the only flu vaccine available came in multiple dose vials that had some preservative.
The amount is tiny. ( less than you would get from eating a tuna fish sandwich) However, why put any substance in our bodies if we can avoid it? We were delighted when they came out with the preservative free version.
So far this season ( as you know) we have not had an adequate supply of the H1N1. We have seen NONE of the preservative free so far.
The government has gone as far as issuing a Thimersol exemption. It is permissible for pregnant women and young children to get the flu vaccine from the multi dose vials.
Children under three will get .25 cc rather than the full dose which is .5 cc

Q. what happens if my child had a shot but now when it is time for the second dose only the mist is available

A. ideally we would like to give 2 of the same type of vaccine, but they can be mixed and matched

thank you for your patience

This flu season has been unprecedented.
We are faced with lots of flu cases and limited supply of vaccine.

Currently we have PLENTY of seasonal flu vaccine for children less than 3. This is preservative free.
We have a small amount of seasonal flu vaccine for the over 3 crowd. We have no more seasonal flu mist and no hope of getting any more of that this year.
As far as I can tell, we are NOT seeing any seasonal flu yet this year.

We are seeing tons of H1N1 cases.
SO far none of our cases have been serious ( although we have had plenty of miserable kids)
The typical case is 3 to 4 days of high fever, decreased appetite, some congestion, some vomiting.

We hope to get more H1N1 vaccine within the next few weeks.
We are as frustrated as all of our patients regarding our lack of adequate vaccine at this time

In an effort to communicate more easily with our patients it has been suggested that we start a blog.
We may try to use this to let folks know about the availability of the elusive flu vaccine
